Custom Order or Additional Item Request Form

Provide detailed information and notify me of any deadlines for the project. Be aware that there may be other orders in queue, and I will inform you of the timeline within 24 to 48 hours of receiving your order.

Once confirmed, I will send you an invoice for payment. For custom pieces, I require 50% payment at the time the order is placed and the other 50% once the item is completed. All items must be paid in full before item is shipped.
